I realize this is a release of my legal rights. The Running Store Running Club,
LLC (TRSRC) includes all its volunteers, staff, officers, agents, and assigns.
By acknowledging below using the supplied check box, I freely choose to have my
child participate in the TRSRC. I understand the dangers and risks of running
practicing or training in any athletic activity include, but are limited to:
death, serious injury to virtually all bones, joints, ligaments, tendons,
muscles, and potential to aggravate other health issues diagnosed or
undiagnosed. I certify that I have consulted with a medical doctor with regard
to my child's medical needs. I affirm that there are no health reasons
precluding my child from participating with TRSRC. I release TRSRC from all
liability for any bodily injury or damage my child may sustain as a result of
participation. Knowing the risks described and in voluntary consideration of
permission to participate with TRSRC, I agree to release, indemnify, and
defend TRSRC and their sponsors from and against any claim of damages or
injuries arising from my child's participation with TRSRC. In consideration of
my child's participation in the Running Club, I hereby grant permission to
TRSRC and The Running Store, Inc. to use any photographs, video, recordings,
or written statements associated with the program. I indicate by my
acknowledgment below that I have carefully read these terms and conditions,
and acknowledge that I understand them fully.I understand and agree to the terms of the above release.
